After signing up below be sure to download and print the forms below. These should be completed and brought with you on April 13.

• Incoming Freshmen & Transfer Students: Physical and Medical History Form
  If you have already gotten your physical, we can accept other branded forms with the same details. 
• Current Ursuline Students: Log Into Magnus & Complete the Athletics Form
